School just let out, but it’s the best time to get school-required immunizations. Your child may need vaccines before they start school next year - don’t wait until August to find out what your child needs. Check your child’s immunization records from home and then use the vaccine voucher below to cover the administrative costs.
To prepare for the 2019-20 school year, Immunize Nevada, in partnership with Carson City Health and Human Services and the Southern Nevada Health District, has created a back-to-school vaccine voucher to assist families with obtaining school-required immunizations. This voucher can be redeemed at any participating CCHHS or SNHD locations and will cover administration costs associated with school-required vaccines, when redeemed before June 30, 2019.
To begin, first check your immunization record through Nevada WebIZ by visiting https://izrecord.nv.gov/, and fill out the required information regarding the child. If you have questions about accessing an immunization record, or need assistance, call the Help Desk at 775-684-5954 or email .
To redeem the voucher, show the healthcare professional administrating the vaccine the voucher, either printed out or in a digital format, and that will ensure the administration fee is waived. If your child needs vaccines in Carson City, Nevada or in Clark County, Nevada, you can redeem it at any of the participating locations listed below before June 30, 2019.
