Immunization Records
If you have not yet accessed WebIZ, you may need to call the Nevada WebIZ Heal Desk to associate your child's Social Security Number with their record. The Nevada WebIZ Help Desk can be reached at 1-877-NV-WebIZ (877-689-3249) or email them at if there are any issues accessing immunization records in WebIZ. A record obtained through Nevada WebIZ is an official record and can be used as proof of immunization for school entry, summer camp, employment, etc.
