Back to School Walk-In Immunizations (Crescent Valley)

 Immunizations will be available to children between the ages of birth through 18 year of age who are meet the eligibility requirements for the Vaccines for Children (VFC) program.  Children must meet the following criteria:
*         Are enrolled or are eligible to enroll in Medicaid,
*         Have no health insurance,
*         Are American Indian or Alaskan Native, or
*         Are underinsured. (Underinsured defined: A child whose health insurance plan does not include vaccinations; health insurance covers only select vaccines; health insurance caps the vaccine cost at a certain limit;
 
Parents or guardian must provide a copy of the child's immunization record at the time of the visit. The parent's picture ID and copy of Medicaid or insurance card are also recommended. Appointments are not necessary.
