2012 Nevada Health Conference Speaker Information

Thank you for presenting at the 2012 Nevada Health Conference November 1 and 2. As we get closer to the conference there are several items we need from you to help keep us on track. The following describes what we require, and a CHECKLIST is also available to help you meet the upcoming deadlines. Please submit all materials to Emily Stratton at 775-870-4336 or . SPEAKER FORM:
All speakers must complete the online speaker form and submit it on or before, Monday, Sept. 24. The information you provide will be used in the conference program and website. Fill out form HERE.

HEAD SHOTS:
Please e-mail your head shot in a jpg to Emily Stratton at on or before Monday, Sept. 24.

HOTEL ACCOMMODATIONS:
All out of town speakers staying at the Tuscany Suites must complete the Hotel Accommodations form on or before Monday, Sept. 24. We will make your reservation and cover the cost for a two (2) night stay. Should you like to extend your stay, please make arrangements separately by calling 877-887-2261. Fill out the form HERE.

CME/CEU ACCREDITATION:
In order for attendees to receive CME and CEU accreditation, please complete these two forms by Monday, Oct. 1 – The “Required Educational Content Form” and the “Faculty Agreement and Disclosure Statement”.

SPEAKER PRESENTATIONS:
Please provide your PowerPoint presentation via e-mail to by Monday, Oct. 15. The presentations will be posted online prior to the conference and we will print limited copies for attendees during each session.

TRAVEL REIMBURSEMENT:
All speakers traveling to Las Vegas will be reimbursed after the conference. We will contact you directly and provide a travel reimbursement form for you to complete, which will be due Monday, Dec. 3. Please note, all speakers who are not local will received a $150 per diem reimbursement along with your flight reimbursement.
