Health Plan of Nevada’s Medicaid Plan and UNR Family Medicine Center Invite you to a Child Wellness and Immunization Fair

Make sure your child is healthy and at their best before flu season. Health Plan of Nevada has teamed up with UNR to provide immunizations and well-child visits. Well-child visits not only assess physical growth and development but also emotional health. Children may be feeling the affects of the pandemic. Talk to your doctor today about resources available to help.

Appointments are preferred for well-child visits but walk-ins are welcome. To make an appointment for your child, call 775-784-1533.

After completing a wellness check and/or receiving immunizations, stop by the HPN Medicaid table for prizes. If you have any questions, please call Member Services at 1-800-962-8074, TTY 711.

Health Plan of Nevada’s Medicaid members who complete their well visit may be eligible to receive a gift card.*

Need a ride? MTM, Inc. can provide same-day transportation for those members who qualify. Call toll-free 1-844-879-7341, TTY 711.
